Instructions
Use this form to guide your plan for a clientÂ’s course of treatment. Review each section’s instructions and
refer to them as you complete the blank form using your client’s information.
The treatment plan is like a map:
•
•
•
Goal = the clientÂ’s desired destination
Objectives = the path to get to the clientÂ’s desired destination
Strategies and Interventions = the counselorÂ’s tools and techniques to use in session to
guide the client to their desired destination; these should be from a theoretical
perspective that matches the theory used in the case conceptualization

Consider the following when writing goals for a client:
•
•
•
•
•
What is the clientÂ’s desired outcome of treatment, in their words?
After collaborating with the client to assess their situation, what have you decided that they need
to do?
What strengths and skills does the client have that they can use to meet their goal?
What outside supports, services, or coordinated plans are needed to meet these goals?

overall daily functioning, and they will discuss these strategies in
each session.
Example: Client will report reduced symptoms of depression,
improved sleep, and improved sleep, and will report improved

Objectives
Break down the goals into smaller concrete steps. What are a few steps that clients will accomplish to
move them toward their goals? List at least 2 objectives for each goal.
•
Example: Client will learn and implement behavioral strategies to overcome depression,
including, but not limited to:
o Daily journaling (e.g., Daily Record of Dysfunctional Thoughts),
o Engaging in at least 2 self-care activities each day (e.g., healthy eating, physical activity,
maintaining hydration, restful sleep), and
o Engaging in at least one meaningful activity each day (e.g., listening to music, spending
time with family and/or friends, coloring).
Strategies/Interventions to Achieve Goals
What are you, as the counselor, going to do with your client to help them achieve the goals and
objectives? Use a theoretical perspective to develop your strategies and interventions. You may use the
same theory you applied in the case conceptualization.
•
Example: Assist the client in developing skills that increase the likelihood of deriving pleasure
from behavioral activation. Explore, address, and resolve underlying assumptions, beliefs, and
biases that support depression. Explore experiences from the clientÂ’s past that may contribute to
current symptoms, and help the client process and resolve these feelings.
